New Patient Registration
Our area of operation is defined by an 8 mile radius around each surgery: This encompasses Tilstock, Bettisfield, Lyneal, Cockshutt, Burlton, Harmer Hill, Weston, Stanton and Higher Heath. This enables us to offer an efficient and quick service for both Home Visits and Emergencies. To be able to give best service to our existing patients, we have a strict policy of not registering anyone who lives or moves outside the area.
If you are unsure you're within our catchment area please give us a call and we will confirm your eligibility. Once you are registered you will be able to take full advantage of all the services that we offer.
To enable us to speed up your registration, please fill in all relevant details on the registration form. Your notes can then be requested from your previous doctor/practice.
You may print off a registration form, fill it out and bring it in with you on your first visit to the practice.
When you register you are also asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. There is an online version of this file too, which you can also print out, fill in and bring to the surgery when you first visit. You will be asked to sign this form to confirm that the details are correct.
